someone messed up loading the mag. and when it didnt shoot they racked the slide and the 45 got shot with the 40 in the barrel.

Glad he's okay, expensive lesson but at least he didn't lose any digits.

So it was an obstructed barrel -- the obstruction just happened to be a complete 40 S&W cartridge... Other than a slight bend to the dust cover of the slide, it apears ok too. I would think that could be straightened.

Gotta pay attention to what is being loaded at all times and keep ammo separate.

I'd say that's a 10+ on the pucker factor. Glad it was only the firearm that sustained damage!

There is a lot to be said about stop firing if something does not seem correct.
