    someone messed up loading the mag. and when it didnt shoot they racked the slide and the 45 got shot with the 40 in the barrel.

    So it was an obstructed barrel -- the obstruction just happened to be a complete 40 S&W cartridge... Other than a slight bend to the dust cover of the slide, it apears ok too. I would think that could be straightened.

    I'd say that's a 10+ on the pucker factor. Glad it was only the firearm that sustained damage!

    There is a lot to be said about stop firing if something does not seem correct.

    Sorry for the loss, but training may have prevented that.

    A little Flitz and Mothers Mag Polish and she'll be good as new. Glad no one was hurt.
